Operative Nuances Far-lateral Approach to the Craniocervical Junction

Received, October 28, 2004. Accepted, March 9, 2005. THE FAR-LATERAL APPROACH is an extension of the standard suboccipital approach, designed to maximize exposure of the lateroventral craniocervical junction. Following a basic principle of cranial base surgery, the angle of view is increased by bone removal. Bone removal involves the most lateral part of the inferior occipital squama and the po...

Schwannoma of the craniocervical junction: surgical approach of two cases.

We report two cases of craniocervical junction schwannomas with a special focus on the surgical approach. The patients underwent a far-lateral approach in the sitting position that facilitated the lesion removal. This report is meant to improve the understanding of this surgical technique as well as improve awareness of its usefulness for similar cases.

Surgical approaches to the craniocervical junction for the resection of chordomas.

The anterior aspect of the craniocervical junction consists of the lower clivus (beginning at the level of the jugular foramen) and C1. Surgical approaches to the craniocervical junction have historically been one of the most challenging and highly morbid procedures (10, 17, 25, 26, 37). The challenge of obtaining adequate surgical exposure in a small operative corridor coupled with the anatomi...

Tuberculosis of the craniocervical junction.

Tuberculosis of the craniocervical junction is rare even where the condition is endemic. It poses problems in both diagnosis and management. We describe 25 cases followed over a period of 12 years, and relate the presentation, diagnosis and management. Of our 25 patients 16 were managed conservatively and nine by surgery. In order to diagnose this condition a high index of suspicion and advance...

Resection of Bilateral C1 Neurofibromas Using a Unilateral Modified Far-Lateral Approach

Resection of foramen magnum lesions through a lateral approach can be technically challenging. Surgical avenues to foramen magnum lesions include a suboccipital craniectomy with C1 laminectomy, a modified far lateral approach with little condylar drilling, or via an anterior approach. The authors report their surgical experience in the treatment of a 36-yearold male with bilateral C1 dumbbell n...
